If you have been in a traffic accident in the Village of Mt. Horeb and...
  • your vehicle or property sustained $1000.00 or more in damage,
  •  
    or if anyone was injured,
  • or if there was $200.00 or more in damage to government property (other than vehicle),

you need to complete the Wisconsin Department of Transportation's MV4002 form, Driver Report of Accident, and send it to the Wisconsin DOT PO Box listed on the form.

 

This document can be obtained at the Mt. Horeb POlice Department located at 222 E. Front Street, Monday through Friday, 8:00 a.m. to 4:30 p.m.
